Please complete the following Enrolment form below to the best of your abilities. Note that each question with a * needs to be completed otherwise you will not be able to submit the form. 

Once you have completed filling in all the required fields and before clicking the ‘SUBMIT’ button please make sure you have clicked and read this POPIA Consent. Once you have clicked the ‘SUBMIT’ button on this formit is considered you have read and understood our terms with regards POPI Act and consent to us having and using your personal information as well as give us the legal consent to communicate with you effectively. 

Please ensure that once your had read and understood the POPIA consent you return to this page to click the ‘SUBMIT’ button otherwise we will not receive your application. To confirm receipt of your application you can send an email to admin@modelm.co.za with your name and surname and the words ‘Online Model Registratioin’ and we can confirm receipt thereof.  

Should you not get a response within 5 working days, consider your application unsuccessful.
